1)  Go to the online payment page (credit or debit card, bank account)
      MBI uses PayPal for safe and secure online payments. You DO NOT need to
      have a PayPal account to use this option.   MBI does not see or have any access       to bank card or account numbers used for this payment method.
      

2)  Download and mail a membership application form with payment
          to the address on the form.
